Pilot lost awareness, the plane continued to sit on autopilot until the crash killed 4

  • Almost two years after a private jet crashed in Virginia and killed four, the National Transportation Safety Board published its final report on the accident
  • Although the investigators were not able to determine a probable reason for the crash, several questions remain unanswered
  • The victims were Adina Azarian, 49, her daughter Aria, 2, Arias Nanny, Evadnie Smith, 56, and the pilot Jeff Hefner, 69

The National Transportation Safety Board (NTSB) published its final report after an examination of a plane crash in June 2023, in which four people were killed.

On Wednesday, May 14th, the NTSB showed that the probable cause of the crash was “inability to pilot due to the loss of cabin pressure for indefinite reasons”. In addition, a contribution factor was listed as a “pilot and owner/operator decision to operate the aircraft without additional oxygen.

“Based on the available information, it is likely that the aircraft consumers due to an oxygen lack during the flight and were put out of action wrote that the symptoms of height -related hypoxia are often vague and are experienced differently by differences of differences”, but if they have not been remedied or suspected, they can lead to death.

At the time of the crash, Adina Azarian, 49, her daughter Aria, 2, Arias Live-in-Nanny, Evadnie Smith (56) and the pilot Jeff Hefner (69) were.

The investigators could not find out exactly whether this was something that happened quickly or for some time.

The NTSB wrote that the pilot contacted the Atlanta Air Route Control Center in the Atlanta Air Route shortly before 1:15 p.m. on June 4, 2023 shortly before 1:15 p.m.

The investigators say that it is likely that the pilot is out of operation when climbing to this cruise height and that the aircraft was probably managed by Autopilot for the rest of the flight until Autopilot was no longer able to maintain control.

After the plane reached a height of 34,000 feet, it flew on at this height – and about an hour later it attacked its intended goal in Long Island and turned over Washington, DC

About two minutes before the crash, the United Air Force pilots were authorized to contact the plane – which led to A “Sonic Boom” by many heard nearby – and they watched the pilot to be sunk on his seat. In addition, none of the other passengers were observed.

Then, at 3:22 p.m., the plane entered “into a rapidly descending right spiral call” and landed in a forested area near Montebello, VA.

An investigation showed that the maintenance staff found a number of problems in the days and weeks before the crash, including “several in connection with the pressure and environmental control system” and a lack of oxygen mask with the pilot page.

The report found that there was no evidence that these problems were solved before the flight.

According to the NTSB, the pilot had medication conditions, including high blood pressure and cholesterol, and were prescribed medication that could lead to impairment. However, they found “no evidence” that the pilot had used its medication inappropriately at the time of the crash.

“Based on the circumstances of the accident,” wrote the NTSB “,” it is likely that all aircraft residents due to a common environmental conditions, such as.
